Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

Duties

• The purpose of the position is to provide comprehensive mental/behavioral health care including primary care integration to a complex patient population
• This involves coordinated care with an emphasis on preventive measures as well as treatment of chronic diseases
• The NP will function interdependently with physicians and will be responsible for providing quality care - consultation - education and collaboration with other healthcare professionals
• The duties include - but are not limited to: Managing and evaluating a panel of patients under his/her care
• Performing comprehensive initial patient mental health intakes - routine follow-ups - preventive and wellness care - as well as spectrum of acute - episodic - chronic and end of life care
• Performing and documenting history and physical examinations
• Providing clinical assessments - diagnoses - and developing plans of care
• managing basic - acute - and chronic conditions
• Requesting consultations from other healthcare providers
• Promoting health maintenance and disease prevention through patient counseling - education - and vaccine administration
• Participating in clinical research or quality improvement projects - instructing health care trainees and non-advanced practice duties as needed
