Job Description

The Bedford-Stuyvesant Family Health Center (BSFHC) The Bedford-Stuyvesant Family Health Center (BSFHC) is committed to advancing health equity and eliminating barriers to care for communities disproportionately impacted by HIV, STIs, and other health disparities. Through funding from the New York City Council Ending the Epidemic (ETE) initiative, BSFHC will strengthen community-based outreach and education to increase awareness of HIV prevention, particularly Pre-Exposure Prophylaxis (PrEP), while ensuring culturally responsive support for underserved populations.

The PrEP Outreach Health Educator will engage community members—particularly men who have sex with men (MSM) of color, women of color, LGBTQ+ individuals, and other priority populations—through field-based outreach, health education, and linkage to services. This role is essential to reducing stigma, increasing PrEP awareness, and ensuring that individuals are connected to prevention, testing, and treatment services.

Required Field work about 75% of the time and take mass transit to travel throughout Brooklyn, NY.

Grant-funded: Contingent upon continuation of grant funding from year to year.

Key Responsibilities To Include But Not Limited To

  • Conduct community-based outreach (approximately 75% field work) across Brooklyn to identify and engage individuals at risk for HIV.
  • Provide culturally competent PrEP and HIV/STI prevention education at community events, venues, and partner organizations.
  • Link individuals to BSFHC’s PrEP, HIV testing, STI screening, and supportive services.
  • Collaborate with community-based organizations (CBOs) and health providers to build referral networks and maintain an outreach database.
  • Re-engage individuals who have fallen out of care or missed appointments through phone calls, texts, and other communication methods.
  • Support implementation of HIV/PrEP education campaigns, including social media and digital outreach strategies targeting hard-to-reach communities.
  • Represent BSFHC at health fairs, community forums, and coalition meetings to increase visibility and strengthen partnerships.
  • Document outreach activities, client interactions, and service referrals in required data systems for reporting and evaluation.
  • Collaborate with clinical and program staff to ensure clients receive ongoing support to overcome barriers to PrEP initiation and adherence.

Education/Experience Required

Perform additional duties as requested by the Program Manager

  • Bachelor’s degree required; equivalent experience in community outreach and HIV prevention may be considered.

Knowledge, Skills & Abilities

  • Strong understanding of the cultural and social factors impacting communities of color, LGBTQ+ individuals, and other priority populations in Brooklyn.
  • Knowledge of HIV prevention, including PrEP, PEP, HIV/STI testing, and harm reduction strategies.
  • Ability to build rapport with underserved and hard-to-reach populations.
  • Strong communication, outreach, and public speaking skills.
  • Ability to work independently while collaborating effectively with a team.
  • Proficiency in maintaining accurate documentation and using data systems for reporting.
  • Experience working with HIV/AIDS, LGBTQ+ communities, and communities of color required.
  • Familiarity with NYC systems of care and local CBO networks preferred

Reports to : Program Manager, Program Director

Annual Salary: $50,000
